The Original Sales Receipt must be mailed in with this form and must clearly show the purchase of qualifying products between November 14, 2008 and December 31, 2008. If your country requires you to hold the original receipt to maintain your warranty, you can send us a copy of the sales receipt that clearly shows the date of purchase and the item purchased.
